How we handle information from this website.
This policy covers what mountainpeaksfamilypractice.com collects and how we use it. Your medical records are protected separately under HIPAA and our Notice of Privacy Practices.

Introduction
This Privacy Policy explains what information Mountain Peaks Family Practice ("Mountain Peaks," "we," "us") collects through mountainpeaksfamilypractice.com, how we use it, the choices you have and how to reach us. It covers information collected on this website only. It does not cover information you share with us as a patient; that is governed by our Notice of Privacy Practices, which describes your rights under HIPAA and how your medical records are protected.
Please read this policy carefully. By using this website you accept the practices it describes.
What we collect
Personal information means anything that can identify you, such as your name, home address, email address or phone number. In general you can browse this website without telling us who you are.
We collect personal information only when you choose to give it to us, for example when you:
- join the membership waitlist (name, email and phone);
- send a message through the contact page or by email;
- call the office or ask us to call you back.
Please do not send medical information through this website. Health questions, symptoms, medications and records belong in a visit, a phone call or the secure member app, not in a web form or email.
Usage information. When you visit, our hosting and analytics tools may automatically record your IP address, browser and device type, the pages you view, the links you click and the date and time of your visit. We may associate this with personal information you have given us.
How we use information
We use the personal information you give us to:
- contact you about the membership waitlist and the opening of enrollment;
- answer your questions and respond to your requests;
- schedule visits and manage membership once you enroll;
- improve this website and understand how it is used, usually in aggregate.
We will send you marketing or newsletter messages only if you have agreed to receive them, and you can opt out at any time by replying or contacting the office. Waitlist details are used to contact you about enrollment, not to build marketing lists.
The member app and patient portal
Members manage visits, messages, records and billing through a secure patient portal and mobile app operated by our practice management partner. Information you enter there is protected health information held in your medical record. It is governed by our Notice of Privacy Practices and the partner's business associate agreement with the practice, not by this website policy.
Security and retention
We use reasonable administrative and technical safeguards to protect the personal information collected through this website. No internet transmission is completely secure, which is one reason we ask that nothing medical be sent through the site. We keep waitlist and contact information as long as needed to respond to you and to open enrollment, after which it is deleted or added to your patient record if you become a member.
Children
This website is intended for adults. We do not knowingly collect personal information from children under 13 through the site. Parents and guardians enroll children in membership and manage their care through the member app.
Third-party websites
This website links to third-party websites, including app stores, Google Maps and our patient portal partner. We are not responsible for the privacy practices of those sites and encourage you to read their policies.
Your choices
You may ask us to correct or delete the personal information you have provided through this website, or to remove you from the waitlist, by calling the office or using the contact page. Requests about your medical record are handled under the Notice of Privacy Practices.
This website is operated in the United States. If you visit from elsewhere, your information will be transferred to and processed in the United States.
Changes to this policy
We may update this policy from time to time. The latest version is always posted here with its date. Your continued use of the website after a change means you accept the updated policy. We will use your information in a manner consistent with the policy in effect when you provided it unless you agree otherwise.
