The content on this website is general information about the practice and its membership. It is not medical advice, does not create a physician-patient relationship and is not a substitute for a visit with your own provider. If you are experiencing a medical emergency, call 911.
Articles in the Health Resources section are educational. Decisions about your care should be made with your provider during a visit, by phone or through the member app.
Mountain Peaks offers Direct Primary Care membership. Membership is not health insurance, not a health plan and not a substitute for insurance. Members should maintain appropriate coverage for hospitalization, emergency care, specialists, imaging and other services outside the practice. The terms of membership itself, including fees, cancellation and included services, are set out in the membership agreement you sign when you enroll, which controls over anything on this website if the two differ.
Pricing and availability shown on this website may change. Enrollment is subject to the practice's capacity to accept new members.
Joining the waitlist does not create a membership or a physician-patient relationship and does not guarantee a place when enrollment opens. The information you submit is used as described in our Privacy Policy. Please do not send medical information through any website form or email.
Members access scheduling, messaging, records and billing through a secure patient portal and app provided by our practice management partner. Use of the portal and app is subject to that partner's own terms and privacy notice, which are presented when you create your account. You are responsible for keeping your login credentials confidential.
